Parents please complete this one question in consultation with your daughter.  Each year we ask all seniors to write their name as they wish it to appear on their diploma and all graduation publications.  This will be done in class at school and will be hand written by the student and include a phonetic spelling too to help Mercy staff with pronunciation at the services at the end of the year!

We are asking that you also complete this form in order for us to confirm the correct name as many of our girls have variations they use interchangeably.  For example Kathleen might be known as Kathy and she may want to use this version of her name, however you may decide together that her proper given name should be used, or you may agree to use Kathy.  In addition some families want to include their middle name and others do not.

Please note, for girls with both a Chinese and an English name, our protocol has been the use the Chinese name first, followed by the American name as a middle name i.e. Peiying Christine Wu.  However please let us know below what you prefer. In addition if your daughter's name requires an accent please be sure that is included.  If you find it difficult to add the correct accent, or have any other questions, please just email me. csmall@mercyhsb.com.  Thank you!
