Athletic  Forms

All Mercy students, whether playing a sport or not, are required to register on the Appryse system for their health information. Once you create a log on, you can download the health form needed. 

What is Appryse?

Mercy High School is excited to partner with Appryse. Appryse is a web-based solution used to fill out medical history, emergency contact information, release forms and Doctor's Physical forms.

As parents, you will now be able to enter all information online and upload or fax doctor physical forms, if necessary. Information regarding your son or daughter’s medical history can be updated throughout the year as it changes. In the event of an athletic injury, you will automatically be notified via phone, email and text message, simultaneously. During the school day normal contact procedures will be followed.

For those parents that already completed and turned in the Athletic Participation forms for their daughters, we ask that you enroll and complete the forms on Appryse after July 13, 2015 when the forms for the new school year will be available. It is only required that you answer the questions and complete the forms with an electronic signature. Mercy will upload your daughter's physical examination form for you.  After signing up on Appryse, you can review and update your information each year plus receive medical emergency notifications through Appryse via phone, email and text.

In order to tryout for a sport this school year you MUST enroll your daughter in Appryse by clicking here after July 13, 2015 when the forms for the new school year will be available.  The required physical form can be downloaded from the Appryse system once you have enrolled.

Enrolling in Appryse

For the 2015-16 school year all families must enroll in Appryse. This may be done starting May 1, 2015. Additionally, all new students and students who wish to particpate in athletics will need to have a Doctor's Physical form on file.

New Students and Returning Athletes

All returning students


  1. Make an appointment with your physician to have a physical evaluation exam.
     
  2. Complete the Appryse enrollment and print out the Doctor's Physical form with your unique Appryse bar code.
     
  3. Take the Doctor's Physical form with you to your doctor appointment. Once completed by your physician you must upload or fax the Doctor's Physical form to Appryse. Note: This is the only form recognized by the Appryse system. Do not upload any other documents as they will not be attached to your records.


1. Complete the Appryse enrollment. That is all!

 Privacy and Security

Appryse is committed to privacy and security. You can read their privacy policy onlineAppryse follows all HIPAA guidelines for security:

  • Appryse runs behind firewalls to protect is servers.
  • All private medical history for students is encrypted in the database. 
  • All communication with Appryse from browsers and mobile devices uses SSL encryption.


 

Parent Drivers:  Forms needed to drive students 
In order for parents to drive students to competitions, you must turn in the following forms with a copy of your driver's license and a copy of your insurance policy, stating the limits you have on your policy.

1. DMV Release of Driving Record
2. Drivers Liability Form 

Once these are turned back in Mercy will submit the DMV form to the DMV for clearance.  This process usually takes over a month to hear back from the DMV. You are NOT cleared to drive until we hear back from the DMV .  You will be notified by Mercy when you are clear to drive.  This DMV form is only due once in your career at Mercy.
