Forms, Photos & Athletic Qualifications

All Mercy students intending to try out or participate in an athletics team are required to complete the online registration and forms on the Magnus Health Portal, along with a Mercy pre-participation physical exam form completed by a physician. Students will not be allowed to participate until cleared by the Mercy Certified Athletic Trainer. 

Steps to Participate in Mercy Athletics

Register on Magnus Health Portal

Registration and form completion can be accessed through the Parents PowerSchool account. Forms should be completed by the student’s parent/guardian. Should any technical difficulties arise with submitting forms, contact Magnus Health Portal directly.

*Every student, regardless of whether or not she plans to play a sport, is required to register on Magnus* 

The following forms must be completed on Magnus Health Portal for full clearance to participate in Mercy Athletics:

  • Vital Health Record
  • Athletic Contract
  • Consent to Treat
  • Concussion Protocol Acknowledgement
  • Sudden Cardiac Arrest (SCA)
  • Annual Physical Exam Form (See “Mercy Pre-Participation Physical Exam” below for requirements. Physical should be uploaded in Magnus once completed.)

Upload Mercy Pre-Participation Physical Exam

All Mercy athletes are required to have a Mercy Pre-Participation Physical Exam Form uploaded in the Magnus Health Portal prior to clearance for participation. No other forms or physician notes will be accepted. We recommend bringing the form to the student’s appointment for the physician or health care provider to complete. Physicals are valid for 365 days from the date signed by the physician. We highly recommend physicals are completed between June 1 and August 1 for the student to by fully eligible through the entire school year and avoid having to schedule an appointment mid year. All athletes must have a physical on file to be eligible to participate in any Summer Strength & Conditioning sessions and open gyms.

The Mercy Pre-Participation Physical Exam Form must be completed by either a physician (MD or DO), Physician Assistant (PA), or Nurse Practitioner (NP). All physical forms must be dated, signed, and stamped by the health care provider. The form will not be valid if it is not dated, signed, and stamped.

Complete Baseline Concussion ImPACT Testing

The ImPACT Test is Immediate Post-Concussion Assessment and Cognitive Testing. It is used to help determine the athlete’s full recovery and readiness to return to her sport and activities. ImPACT evaluates multiple aspects of neurocognitive function including memory, attention, brain processing speed, reaction time, and post-concussion symptoms. ImPACT provides specific objective information regarding the severity of the injury and can help take the guesswork out of the return to play decisions as well as prevent the cumulative effects of multiple concussions.

The following individuals are required to have a Baseline ImPACT Test on file with Mercy and should sign up with the link below:

  • All incoming Freshman planning to participate in a sport during the 2019-2020 school year.
  • All incoming and returning Cheer student-athletes.
  • All incoming and returning Dance student-athletes.
  • Returning student-athletes who have not taken a Baseline ImPACT Test while at Mercy (if you are unsure if you have a test on file, please contact Keri Sotak at ksotak@mercyhsb.com.)

Team Photos

Team and individual photographs are taken and made available to purchase by Lifetouch. Lifetouch is currently in a transition between order platforms, and so we will use a temporary process for families to order Athletic photos:

Lifetouch's Pre-Order site is available HERE. You will be able to place orders up to 2 weeks before and after the team photos are taken. Athletes can also purchase packages on the day of team photos, as the photographers will have these FORMS available. 

*Please NOTE - Buddy pictures cannot be ordered online, and will need to be purchased the day of the photos session, payable by cash, check or credit card. Each individual that wants a copy of the image must fill out an order form on that day (example attached above).
