Forms & Athletic Qualifications

Registration and form completion can be accessed through the Parents PowerSchool account. Forms should be completed by the student’s parent/guardian. Should any technical difficulties arise with submitting forms, contact Magnus Health Portal directly.

*Every student, regardless of whether or not she plans to play a sport, is required to register on Magnus* 

The following forms must be completed on Magnus Health Portal for full clearance to participate in Mercy Athletics:

• Vital Health Record
• Athletic Contract
• Consent to Treat
• Concussion Protocol Acknowledgement
• Sudden Cardiac Arrest (SCA)
• Annual Physical Exam Form (See “Mercy Pre-Participation Physical Exam” below for requirements. Physical should be uploaded in Magnus once completed.)

All Mercy athletes are required to have a Mercy Pre-Participation Physical Exam Form uploaded in the Magnus Health Portal prior to clearance for participation. No other forms or physician notes will be accepted. We recommend bringing the form to the student’s appointment for the physician or health care provider to complete. Physicals are valid for 365 days from the date signed by the physician. We highly recommend physicals are completed between June 1 and July 8 for the student to by fully eligible through the entire school year and avoid having to schedule an appointment mid year. All athletes must have a physical on file to be eligible to participate in any Summer Strength & Conditioning sessions and open gyms.

The Mercy Pre-Participation Physical Exam Form must be completed by either a physician (MD or DO), Physician Assistant (PA), or Nurse Practitioner (NP). All physical forms must be dated, signed, and stamped by the health care provider. The form will not be valid if it is not dated, signed, and stamped.

The ImPACT Test is Immediate Post-Concussion Assessment and Cognitive Testing. It is used to help determine the athlete’s full recovery and readiness to return to her sport and activities. ImPACT evaluates multiple aspects of neurocognitive function including memory, attention, brain processing speed, reaction time, and post-concussion symptoms. ImPACT provides specific objective information regarding the severity of the injury and can help take the guesswork out of the return to play decisions as well as prevent the cumulative effects of multiple concussions.

Information on ImPACT testing will be sent to the student's Mercy email account prior to try-outs.
